Too hot
Phoenix / Scottsdale / Sedona (95-110 F), Las Vegas, Palm Springs, Tucson, Albuquerque, Moab, St. George / Zion lower elevations, Mexico Pacific resorts. Heat plus dehydration is a real third-trimester risk and the cool-evening relief is unreliable.
Too high or too remote
Telluride (~8,750 ft), Crested Butte (~8,900 ft), Vail / Beaver Creek (~8,150 ft), Mammoth (~7,900 ft), Bryce Rim, Yellowstone interior basins (sustained 7,500+ ft and far from L&D-capable hospitals).

Practical notes

Flying at ~28 weeks

US domestic: most carriers allow travel through 36 weeks. International (Canada): generally fine to 28 weeks; some carriers want an OB letter after 28 weeks.

Get a dated OB letter regardless - cheap insurance.

Altitude

ACOG considers up to ~8,000 ft generally safe for healthy pregnancies on short stays. Acclimate gradually, hydrate aggressively.

Jackson, Whitefish, Bozeman, Banff are clean. Santa Fe, Taos, Park City sit at the upper end.

Hot springs / hot tubs

Avoid water above ~100 F and limit any soak to 10 minutes - core temperature risk in the second/third trimester.

Skip Steamboat, Glenwood, Banff hot springs, Yellowstone pools.

Hospital proximity

Aim for under 45 minutes from a hospital with L&D. St. John's (Jackson), Christus St. Vincent (Santa Fe), and North Valley (Whitefish) qualify.

Tofino is the edge case at ~1h to Port Alberni.
